Johnson Memorial Hospital Strategic Plan 2012-2017
Introduction
Johnson Memorial Hospital (JMH) is a 50 Bed facility that provides health care and promote wellness services throughout the Douglas County area. JMH provides services for both inpatient and out-patient programs originating from its central location which is in Douglasville. The hospital consists of 20 acute care beds, 20 long term care beds, and 10 chronic care beds with one of these beds being dedicated as a respite bed for short stays. The Fulton Regional Health Centre is administered by the Hospital and provides primary care to residents of the Fulton area which is located approximately one hour from the city of Douglasville.
Johnson Memorial Hospital provides:
* Acute care for inpatients and chronic care for long term patients
* Emergency department services 24 hour a day
* Cardiac care services that consist of telemetry, stress testing and cardiac rehabilitation
* Labor and delivery services that are emergencies
* Diagnostic testing
* Rehabilitative services diabetes
* Care for cancer
Due to significant changes within the communities and landscape of the health care system in the past four years the need to modify and develop a Strategic Plan that was a reflection of the present population and patient demographics was necessary. In the last few years the area has lost several major employers with the closing of two manufacturing plants. Following the loss of these major employers labor forced out migration resulting in higher of those that are 45 and older remaining in the communities.
The role of the rural hospital is beyond simply caring for sick patients. It is the opportunity for expansion beyond the traditional hospital role which is facilitated through integrative and collaborative partnerships. These partnerships consist of local and regional health care providers, stakeholders, the communities and the public. Innovative services such as primary care, health promotion, chronic disease management, and specialty care can be delivered by methods developed through partnerships.
The vision, values and strategic directions are outline in this Strategic Plan. It will be use by JMH to help guide them in making decisions as they move forward. The plan provides a description at a high level of JMH's current initiatives and the plan that will be implemented to build a sustainable, high quality, health system. The Plan reflects the input from several individual and groups as well as the knowledge obtained by ongoing monitoring of the external environment (Zuckerman 2011).
Vision
Johnson Memorial Hospital shall serve as the choice of care for both primary care and preventive services for the people of Douglas County while providing exceptional quality care.

Strategic Directions
In the period 2009-2012 JMH will use four directions to guide them. The strategic direction being used comes from the analysis of the population data, consumer and community focus groups, healthcare partners and internal data from the hospital.
The organization has use the mission, vision and values to guide the development of the strategic direction which is intended to move JMH closer to its vision. For each goal with the strategic direction a desired end point has been articulated for the future. Objectives and key performance indicators are used in performance relations to measure goals. The goals were established to provide guidance to senior level staff for using while preparing annual plans and budgets.
Strategic Goals and Objectives
1. Building understanding and raising awareness as it pertains to critical health care issues within the Douglas County community
Activity: Co-sponsor the Health Conference in collaboration with other healthcare organizations within the community.
Objective: A successful conference documented by an evaluation summary that informs planning for the future. In addition to partners being committed to designing and delivering the conference annually.
2. Develop and advocate for a rural health care policy agenda
Activity: Identify key legislative priorities for the Georgia legislative session and advocate for legislative priorities.
Objective: A sheet stating legislative priorities and widely distributed.
