STATEMENT OF WORK

1.Number and Term of Contract Employees: Consultant shall provide services of at least one student and instructor, for the VA Medical Center, for the period of April 1, 2008 through December 31, 2012.

2.Hours of Work: 8:00am - 4:30pm, Monday through Friday, excluding Federal holidays.

3.hysical Presence and Leave Time: Consultants must be present at this facility and performing the duties outlined herein in order to receive payment. Services performed at other than this VA site are not compensable. Hours of service must be documented on time sheets located in the Operative Care Line administrative office in order for him/her to be compensated. Consultants will not receive compensation for any hours such as sick leave or annual leave from the VA. Consultants will not work holidays nor receive compensation for Government holidays from the VA. (NOTE: Holidays observed by Federal Government are New Year's Day, Martin Luther King's Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans' Day, Thanksgiving, Christmas and any other day specifically declared by the President of the United States to be a national holiday.)

4.Benefits/Personnel Policy: The consultant shall assume full responsibility for the protection of their employees in furnishing services under this purchase order (i.e., worker's compensation, health examinations, income tax withholding and social security payments).

5.Consultants shall not be considered a VA employee for any purpose.

6.Consultants shall provide their uniforms.

7.Duties: Review the operating room (OR) flow. This will involve, (1) measuring the current performance of the OR, (2) developing statistical models for forecasting demand for surgeries, (3) conducting analytical studies related to improving scheduling of surgeries in the OR, and (4) conducting studies to develop more efficient rules for resource (equipment and staff) deployment.

The goal is to improve efficiency of the OR by identifying opportunities for improving patient flow through the OR and improving the quality of patient care.

8. Overtime: When applicable, overtime will be paid in 15 minute increments. The overtime rate shall not exceed one and one half times the regular hourly rate of pay.

Payments: Payment will be made monthly, in arrears, upon receipt of a properly prepared invoice.
