1.) The unfavourable weather, which is unfortunate caused the vessels to sink. Further, the inexperienced crew may also be one of the reasons for the said disaster.

Case -II Answers:

1) The communication problem did arise as the foreman, a shy man when speaking to his immediate superiors, mentioned the matter to the senior supervisor in a weak, ineffectual manner. The senior supervisor did not pay attention towards it and nodded his head. He paid much attention towards the report that he was writing instead of paying attention to what the foreman said.

2) The following might have been done in the mentioned scenario to avoid communication problem :

a. The foreman should have clear and complete information which is to be communicated.

b. This information should be specific, meaningful and clear. The foreman should feel that it is his responsibility to communicate about the issues and he should understand the outcomes of those issues. He should not feel shy while conveying the above.

c. Communication cannot be a one-way traffic. Both the communication channels should be kept open. So, the foreman should pay attention and should be clear in what he is going to communicate and in the same way the senior supervisor should pay attention towards the foreman on what he says.

d. If the foreman feels shy on communicating something to his superiors verbally, he should find an alternate media by which he can communicate effectively.

e. The senior superior should develop such a relation and environment such that his sub-ordinates will not feel shy while communicating with him.

f. The senior supervisor should develop listening skills as listening plays a vital role in perfect understanding of a communication. Good listening is good understanding.

g. There must be mutual trust, faith and perfect understanding between the foreman and his superiors in this scenario.

Case -III Answers:

1) How do you view the present fraud case: a human failure or a system failure?

a. In my opinion, present fraud case is not only a human failure but a system failure as well. In this fraud case it is a human failure as the fraud is done intentionally by Venkatraman. But he was able to do it as the system is not managed, checked and monitored on a timely basis. This made Venkataraman to easily steal the drawing books and issue them. If we talk about Venkatraman, he used to make this type of fraud because it was not checked time to time and there is no such security measures taken to avoid such kind of risks / frauds.
