Below are the ten questions that I would ask an individual during an initial assessment of our meeting.
1. What was the main reason you decided to be helped by a therapist?
This question will provide further insight into why they want help, and most importantly what they need help with. In addition, this question can help me to understand what they expect from me as a therapist, as well as what they expect to happen during therapy.
2. What was your relationship with your parents like? Could you describe your relationship with them currently ?
This question will provide further insight as to how the relationships with their parents were during their upbringing, as well as the current state of their relationship as it stands. This question could provide insight should it be related to the reason why they are experiencing problems in their everyday lives.
3. Could you tell me about your past and current romantic relationships? Are there any children that have resulted from any of your relationships ?
This question will provide insight into the romantic relationships that the individual has or have had. In addition, could present the type of relationships the individuals was or is involved in, and if they were abusive in nature. Also, such relationships could be a major cause of stress and depression in an individual's life, and could help with the diagnosis, and treatment of the patient.
4. What makes you happiest about your life currently?
This question helps to set a positive tone, as the previous questions may have cause the patient to divulge information that could have been upsetting and negative to them. Inputting some sort of positive tone to the assessment, could make them more comfortable, and be willing to give more insight into their lives. In addition, I can use what makes them happy to help them realize they are doing well, and as a form of motivation to get them to see that they are capable of happiness.
5. What are you not happy with about your life currently?
This question will help to provide deeper insight as to what is bothering them. In addition, this will help me to create a list of things to potentially work on with the client, and giving them some positive reinforcement into how they can deal with such problems.
6. Do you have any feelings of anxiety, depression, or suicidal thoughts? Does your family have a history of mental health disorders ?
This question will help to provide information as to the individual's current mental state. This information will help to be able to assist the client in treatment, and ensure that if any medication is to be subscribed that it helps
